How-toForms

Build and publish a form

Create a draft, add sections and questions, verify the respondent preview, publish, and obtain its share link.

This guide produces a published standalone form whose respondent view has been checked before its link is shared.

Before you begin

You need:

  • an authenticated organization-admin account with permission to manage forms;
  • the correct organization slug;
  • the questions, required-answer rules, and intended audience;
  • a decision about sign-in and repeat-response limits for the standalone share link.

Do not use confidential customer answers while testing a new form.

Create the draft

  1. Open https://forms.infra.one/your-organization-slug/forms.
  2. Select Add form. Forms creates Untitled form and opens the builder.
  3. In Basics, enter the Form title and optional Description.
  4. Select Save in the Basics card.

Set response rules

  1. In Settings, choose whether to enable Only available to signed-in Infra One users.
  2. If the form will also use recipient-specific links, choose whether those recipients may remain anonymous with Allow anonymous responses.
  3. Leave Limit to one response per recipient enabled unless signed-in open-link responders or recipient-specific distributions may submit more than once.
  4. Select Save in the Settings card.

On the generic Share link, the sign-in setting governs access. A signed-in response is attributable to that user; a logged-out response is anonymous. The one-response setting can constrain a signed-in responder, but a logged-out person can create another anonymous response. Allow anonymous responses controls attribution for recipient-specific links rather than changing that open-link behavior.

Add sections and questions

  1. Select Add new section.
  2. Enter the Section title and optional Description, then select Save.
  3. Under the section, choose a Type, enter the Question, and enable Required when an answer must be supplied.
  4. Select Add question.
  5. For a choice question, add each Add option label and Value.
  6. Open Edit question to add help text, type-specific limits, or a Show this question only when… condition.
  7. Drag sections or questions by their reorder handles when the sequence needs adjustment.

Preview and publish

  1. Select Preview in the footer. The respondent renderer opens in a new tab and records no response.
  2. Check the title, section order, required markers, choice labels, conditional questions, and upload constraints.
  3. Return to the builder and select Publish.
  4. Confirm Publish form.
  5. After the success message Published the form, select Share.
  6. Copy the URL only to the intended audience. Treat the URL and QR code as access-bearing material.

Success check

The form shows Published, Share is enabled, and a fresh preview matches the intended question order and settings. The form also appears in Forms with a response count of zero until someone submits.

Troubleshooting

  • If Add form or Publish is denied, confirm that you are an organization admin in the organization shown in the URL.
  • If Share is disabled, the form is still Draft or is Closed.
  • If publishing does not complete, confirm the form is still Draft and that any open editor has been saved. Publishing changes the draft state; it does not validate the form’s question content, so check that content separately in Preview before sharing.
  • If conditional questions behave unexpectedly, reopen Edit question and verify both the source question and comparison value.
